• Medical Forms - Due June 27, 2023

  Medical Exam & Immunization Form This form must be filled out for each incoming student and returned to Magnificat by Monday, June 26, 2023. Please contact the School Nurse if you foresee an issue with meeting this deadline.    If your daughter has a medical condition, please see below for links to the appropriate medical forms. A doctor’s signature is required for any medication to be given during the school day. Click here for the Magnificat Medication Policy.

  The Clinic's school hours are Monday through Friday, 8 a.m. - 3:30 p.m. Please encourage your daughter to find the Clinic during the first few weeks of school in case she may need something at any point throughout the school year.
